State Cup Reminder - Updated August 24, 2005

Fall 2005 UYSA State Cup will be held for Boys U-15 through U-19. Also this year we will have U-11 boys and girls playing for the title of State Champions but they will not advance to the Far West Regional Tournament. Tournament experience will be useful in applying for out of state tournaments.

All State Cup teams, including the U-11 boys and girls teams, must submit the same required documentation to enter State Cup (see check list below.)

The Fall 2005 UYSA State Cup dates are Monday, October 17- Saturday 29. Registration has started and all registrations must be in to the state office by 5 PM September 2. NO EXCEPTIONS!

STATE CUP CHECK LIST:

1. Read UYSA 2006 State Cup Tournament Rules and Regulations available at: www.utahyouthsoccer.net

2. All teams are required to check-in on Friday, October 14: at the UYSA State Office from 9-5 PM.

3. Check the UYSA web site link every 24 hours for changes or updates at www.utahyouthsoccer.net

4. Prior to check-in, please check to determine if your team has players and or coaches who are ineligible to participate. Details below:
a. Players:
i. No guest players. Only permanent roster players are eligible.
ii. A player cannot be dual rostered in the same age group
iii. If a player is dual rostered in different age groups, they must play for their primary age group
b. Coaches:
i. Coaches must be rostered with, and have a coach ID badge for their team
5. Required for Check-In
a. Teams must present a clear and legible birth certificate for each player. NO exceptions!!

[There have been many questions about the birth certificate requirement for registration. To clarify: You do not need to bring the original birth certificate. You can bring a notarized one. To have your child's birth certificate notarized:
1. Look in the yellow pages of the telephone directory under notaries publis. Additionally, mailbox shops, copy services, and banks often provide Notary services to the public.
2. The notary will make a copy of the birth certificate, notarize it and sign it.
3. Bring the notarized copy to registration]
b. For teams that have players with Foreign Language Birth Documents, they must bring a translated and notarized copy
c. Team Manager Name
d. Team Manager Current Phone Number(s)
e. Coach Name
f. Coach Phone Current Number(s)
g. Copy of players UYSA registration form
h. Player Cards
6. At check-in the Team Managers will be instructed on the procedure for the Game Roster Form where the rosters freeze at 18 players 24 hours prior to game time.
