Notice of State Cup Check-in

Notice of State Cup Check-in State Cup Director Jodene Smith has announced the dates and locations for the mandatory check-in prior to the start of the State Cup Tournament. This mandatory State Cup Check-in will take place at the following:

Utah Youth Soccer Association office, located at 4476 S. Century Drive, Suite D, Salt Lake City, on October 14, 2005, from 9:00 AM to 5:00 PM.

South Davis Soccer Office, located at 600 N. 500 W. Suite C Bountiful, on October 14, 2005, from Noon to 7:00 PM.

Team coaches or managers are required to bring the following items to the check-in:

1. A clear and legible birth certificate for each player. NO exceptions!! After re-visiting the issue, the State Cup committee has decided that it is no longer necessary to have Birth Certificates notarized. A clear, fresh, legible copy will be sufficient.

2. For teams that have players with Foreign Language Birth Documents, they must bring a translated, legible copy.

3. Team manager name and current phone number(s)

4. Coach name and current phone number(s)

5. Copy of players UYSA registration & medical release form

6. Laminated USYSA pass for each player and up to 4 coaches/trainers passes.

At check-in the Team Managers will be instructed on the procedure for the Game Roster Form where the rosters freeze at 18 players 24 hours prior to game time.
