President's Cup Check-In Time Announced

Below is information about check-in for the 2005 President's Cup. Please contact Amy Oakeson at 268-3365, with any questions.

Check in will take place Wednesday, October 19, at the UYSA office, from 2PM-7PM

You must bring:

• Team roster, make sure it is updated if you have made any changes since registering
• Medical Authorization Form for each player
• Birth Certificate for each player
• Player Passes
• Guest Player Form for each guest player
