Reminder of Roster Deadline for Spring 2006 State Cup
Mandatory Coach Meeting Times Announced

State Cup Director Jodene Smith announced, today, that the MANDATORY State Cup Coaches Meeting is scheduled for Friday, May 5, 2006, at the Utah Youth Soccer Association office, located at 4476 S. Century Drive, Suite D, Salt Lake City. The meeting times are set according to age group as follows:

BOYS: U-14: 8:00 PM U-13: 8:15 PM U-12: 8:30 PM

GIRLS: U-19: 8:45 PM U-18: 8:45 PM U 17: 9:00 PM U-16: 9:15 PM U-15: 9:30 PM U-14: 9:45 PM U-13: 10:00 PM U-12: 10:15 PM

Please note for the mandatory coaches meeting:

• You must be listed on the roster as a coach or assistant coach for the team that you are representing.
• No Director of Coaching to represent more than one team.
• One coach or team manager must attend for each team.
• Failure to show or have a representative for each team will result in the removal of your team from State Cup without a refund of your tournament fee.

Smith would like to remind all teams participating in the 2006 Spring State Cup Tournament that all rosters for State Cup will be frozen on Monday, May 1, 2006. On Monday, May 1st at 5:00PM MDST, the online management system will automatically lock any changes or additions to State Cup rosters from being made. This means that any changes must be completed in the online management system prior to this time. Please contact your District Registrar if necessary.

Smith also announced that it is not required to submit a roster prior to the mandatory check-in scheduled for May 12, 2006. This mandatory State Cup Check-in will take place at the Utah Youth Soccer Association office, located at 4476 S. Century Drive, Suite D, Salt Lake City, from 9:00 AM to 5:00 PM. Team coaches or managers are required to bring the following items to the check-in:

1. Final State Cup Roster.
2. Laminated USYSA pass for each player and up to 4 coaches/trainers passes.
3. Medical release forms for each player (UYSA registration form).
4. Birth Certificate on each player (Coaches copy).
