President's Cup Check-In Time Announced

Below is check-in information for the 2006 President's Cup. Please contact Amy Oakeson at 268-3365 with any questions. Check-in will be held on Wednesday, October 11th at the UYSA office, 4476 South Century Dr. (450 West), Suite D, Salt Lake City from 2:00PM-6:00PM.

You must bring:

• Team roster, make sure it is updated if you have made any changes since registering
• Medical Authorization Form for each player (Medical Authorizations are at the bottom of players Registration Form)
• Birth Certificate for each player
• Player Passes
• Guest Player Form for each guest player
