State Cup Entry Deadline Nears
Applications Due at State Office on August 9th by 5:00 PM

The entry deadline for the 2005 State Cup for U15, U16, U17, and U18 boys teams is Monday, August 9th. Applications must be received at the UYSA State Office not later than 5:00 PM. No late applications will be accepted.

Teams must submit the following materials by the deadline:
1. Typed Application form, which is available by clicking here.
2. Typed team roster, which may be printed out from the new online team management system, accessible by clicking here.
3. A check to Utah Youth Soccer for the $400 entry fee.
4. A second check payable to Utah Youth Soccer in the amount of $500 as a performance bond, which will be returned if the team plays all its assigned games at the state and regional tournaments.

Please note that, thanks to the new online team management system, it is no longer necessary to submit birth certificates of the players with the State Cup application. However, coaches should check the online rosters to make certain that the names of the players are correct (and should exactly match the name on the player's birth certificate). If you identify an incorrect name on the online roster, contact your district registrar by e-mail immediately. The Fall portion of the State Cup Tournament will run nearly full two weeks, starting October 4, 2004 and culminating with the championship matches on October 16, 2004.
