Notice of Deadline for Fall 2004 State Cup Application

Fall 2004 State Cup applications, entry fees, and birth certificates are due in the Utah Youth Soccer Association office, at 4476 S. Century Dr. Suite D, Salt Lake City, UT 84123, no later than 5:00 PM, August 12, 2003. Applications and roster forms must be typed or printed. Handwritten forms will not be accepted. State Cup Applications and Roster Forms can be found in .DOC format by clicking here. No applications will be accepted after the deadline, no exceptions!
