Notice of Deadline for Fall 2005 State Cup Application

State Cup Director Theresa Ortiz announced today that due to the expected number of teams participating, the Fall 2005 State Cup Tournament will run nearly full two weeks, starting October 4, 2004 and culminating on October 16, 2004

Fall 2005 State Cup applications, entry fees, performance bond, and birth certificates are due in the Utah Youth Soccer Association office, at 4476 S. Century Dr. Suite D, Salt Lake City, UT 84123, no later than 5:00 PM, August 9, 2004. Applications and roster forms must be typed or printed. Handwritten forms will not be accepted. State Cup Applications and Roster Forms can be found in .DOC format by clicking here. No applications will be accepted after the deadline, no exceptions!
