UYSA e-Travel System
UYSA e-Travel Forms Available:
- Permission to Travel
- Approval for teams to travel to (1) any matches or event affiliated with US Youth
Soccer or (2) any open tournament sanctioned by a USSF affiliate.
- Guest Player Roster
e-Travel System Instructions: (Please read carefully before proceeding.)
General Instructions
To access to the UYSA e-Travel Forms System, click on the e-Travel logo. The UYSA e-Travel Form selection screen will open. After completing the selected form, you will be given an instantly approved form which may be printed directly from your computer. Take the approved e-Travel form(s) and other required forms with you to the tournament/event you are attending. (Note: Your team number is required on the form(s) and is located on your player pass.)
Permission to Travel Instructions
All forms listed below must be completed and in the possession of the team coach, assistant coach, or manager upon attending the event:
- UYSA Approved e-Travel Form signed by the coach.
- A Roster that has been signed, stamped or both, by the appropriate district registrar and includes all players and team officials.
- UYSA Registration form with Consent for Medical Treatment.
By completing the Permission to Travel Form you certify (1) the information provided is accurate, (2) the team is properly registered according to the rules and regulations of US Youth Soccer and Utah Youth Soccer Association, (3) the team complies with all UYSA team and player registration rules, and (4) the team is not under suspension or in bad standing with any organization.
Guest Player Roster Instructions
All forms listed below must be completed and in the possession of the team coach, assistant coach, or manager upon attending the event:
- UYSA Approved e-Travel Guest Player Form signed by the coach.
- UYSA Registration form with Consent for Medical Treatment for each guest player.
- UYSA Specialty Player Action Form with all required signatures for each guest player.
By completing the Guest Player Roster Form you certify (1) the information provided is accurate, (2) the players listed have player passes and are following all UYSA policies regarding tournament guest players, (3) the team is registered according to the rules and regulations of US Youth Soccer and Utah Youth Soccer Association, (4) the team complies with all UYSA team and player registration rules, and (5) the team is not under suspension or in bad standing with any organization.
